Create an address

Full address creation flow:

Calculate the next pathIndex for the segment combination (SQL MAX, retry on a concurrency conflict)
Build the canonical derivation path
evaluatePolicy (action=create_address): 403 on deny/block, 202 + request id on requireApproval (creation sent for signing)
Wallet Service /wallet/raw_address: get the on-chain address
Write the Address + synchronously register it with tron-events monitoring; if monitoring fails, the address is rolled back (all or nothing)
